Permission Form for 16 year olds What you Must Know Informed Consent Medical Research NAT	Blood Drive Permission Forms You can find out more about giving blood in the links to the left.  If you are 16 or if your child is 16 you must sign the permission form on the left and send it to CCS with your child in order for them to donate. This Blood drive is co-hosted by the Champion Christian School Parent Advisory Committee and the Student Council at Champion Christian High School.  Scholarship funds are raised for CCS scholarship funds if enough pints are given.  Please review all of the links attached, print the permission form from the top link to the left, sign it and return it with your child on the day of the blood drive.
