Dear Kindergarten Parents,

      Pennsylvania State Law and Champion Christian School require the following immunizations to be completed prior to entering Kindergarten:

4 doses - Diptheria, Pertussis, Tetanus (DPT) (DT) (fourth dose after age 4)

2 doses - Measles, Mumps, Rubella (MMR) (first dose after age 1)

1 dose - Chicken pox vaccine (VZV) or age child had disease

3 doses - Hepatitis B Vaccine

3 doses - Oral Polio Vaccine

All immunization dates must be verified by

your family doctor and given to the school

nurse before the first day of school.
