FFNA INSTRUCTION SHEET

GENERAL INSTRUCTIONS: Please follow the General Instructions included with your FFNA application. The following instructions are intended to help provide you with information needed to complete the FFNA application.

APPLICATION PAGE 1:

•         Complete name of school is Champion Christian School

•         Champion Christian School ID #2172

•         Champion Christian School complete address: 2166 Indian Head Road, Champion, PA 15622

APPLICATION PAGE 2:

•         Please refer to FFNA instructions.

APPLICATION PAGE 3:

•         Champion Christian School is requiring that your 2009 Income Tax Return be used to complete the FFNA application. As noted in the FFNA instructions, if you have not yet filed your 2009 Income Tax Return you must submit photocopies of the past year’s taxes including any appropriate Schedules.

•         As noted on the FFNA application, if you feel that your financial circumstances have changed and you feel that you need to give an explanation, you may enclose a letter along with the FFNA application. Please see the FFNA instructions for more information.

OTHER IMPORTANT INFORMATION:

•         The cost to process the FFNA application is $20 if sent directly to FFNA. You can complete the application online for $30 ( http://www.ffna1.com ) Both of these ways take a month to complete. You may also pay $40 and get your application processed in two weeks.

•         Remember to include the following: *Notification Card (optional), *Payment & Tracking Envelope, *Letter of Special Circumstances (optional), *Application, & *Most Recent Federal Income Taxes. Include the * items in the large white envelope and send to FFNA. Please review instructions thoroughly on the FFNA application.
